Form of Proposal

Applicants must submit one (1) complete copy of the grant proposal. These proposals should be bound in a secure, but non-bulky fashion. Please do not send loose pages or paper clipped packets. Do not submit grant proposals in three-ring binders or other large packages.

All proposals are limited to five (5) pages in length. This page limit does not include any required attachments.

Submission

Applications for grants must be sent by regular first-class or Priority mail to the address below:

Carrico Family Foundation
PO Box 4436
Medford, OR 97501

Please DO NOT send your application by any form of mail that requires a signature for receipt, such as certified or registered mail.

Frequency of Application

The Foundation receives numerous grant requests and is unable to fund all projects, even though those projects may be well-designed and well-conceived. Applicants may submit only one (1) application per year, whether successful in receiving a grant or not. The Foundation may hold a grant over and fund it at a later date.

Payments and Reporting

Grant awards are paid upon receipt of the signed agreement.

Organizations receiving funds from the Carrico Family Foundation must provide a certified statement that the funds received from the Foundation have been used for the purpose intended. This statement, which must be signed by the non-profit’s Board Chair and Chief Financial Officer (or accounting firm), must be filed with the Foundation no later than 13 months after such funds are awarded. The Foundation must receive this report before another application is considered. Multi-year grants are required to file a Final Report annually until the entire grant has been expended. The Foundation values Final Reports. Failure to report as required by the Foundation can influence future grant decisions.

Carrico Family Foundation is regularly expanding and refining our process and priorities.
